It is that time of year again – forms, forms, forms. I know you love them, but not as much as me. In an effort to try and eliminate confusion, we are grouping forms based on what the scout or adult is participating in this year. Since the BSA Annual Health and Medical Record form is required in all 3 Groups, when you turn that form in, please designate which Group it is for (post-it note, etc). Note that there are forms required for the adults too.
For example, if your scout is only going to summer camp, then you would need to complete Group 1 and Group 2. Even though you turned in the BSA Annual Health and Medical Record form for Group 1, you will still need to turn in 2 complete copies of Parts A, B, and C for Group 2.
If your scout is going to Seabase and summer camp, then all 3 Groups apply to you. Even though you turned in the BSA Annual Health and Medical Record form for Group 1, you will still need to turn in 2 complete copies of Parts A, B, and C for Group 2 and 1 copy of Parts A, B, C and D for Group 3.
There will be a black basket at all meetings on the sign-up sheet tables for collection of the forms. Please do not hand them to anyone but myself (Britt Turner) or Debbie Moore. I will also be at most meetings to answer any questions or feel free to call or email me.

- Adults staying more than 72 hours need to complete Parts A,B,C (less than 72 hours Parts A & B only)
- Element of Danger form for Blue Ridge summer camp (form not available yet)
- BSA Annual Health and Medical Record form Parts A,B and C
- Group 3 DUE 6/12/13
- Seabase High Adventure scouts and adults
- Must complete the following forms:
- BSA Annual Health and Medical Record form Parts A,B,C and D
- New form must be used with 9/2012 revision date
- Includes copy of both sides of Medical card
- Only 1 copy needed
- Adults must be a registered member of our Troop (see Anita Davis for details)
- Adults must have taken Youth Protection training in last 2 years and provide a hard copy of the certificate
