All scouts, both camps:   **Remember – A doctor’s signature is required within the past 36 months.  For those who do not require a new physical, parents will be asked to re-sign the existing form indicating that the health of their son has not changed.

Adults attending summer camp must have a physical form, signed by a doctor.  Those under 40 can use the Class 1&2 form above.  Those 40 and over must use the Class 3 form.
